Support Coordination Agency
Welcome to Family Care of New Jersey. We are here to assist participants and their families in finding and coordinating the necessary services, supports and resources within the community that are required by adults with intellectual or developmental disabilities. We focus on their needs, hopes and dreams.
Family Care of New Jersey began in memory of my Aunt Maria. Aunt Maria had a brain aneurysm at 16. She was completely paralyzed and only had use of one hand. She was in the Division of Developmental Disabilities (DDD) system and attended the 1st Cerebral Palsy Center of Belleville for 54 years. Maria had a fulfilled life. She lived independently, worked, attended social activities, vacationed and was always surrounded by friends and family. Here at Family Care of New Jersey we believe that with the right support coordination agency everyone can have a prosperous life. With the many years of experience that our employees have, the team here at Family Care of New Jersey realizes that with the right support and guidance people can obtain their dreams. Whether it be employment, living situations, community involvement, health goals or so much more. We are fully dedicated to making your dreams and goals a reality. We do this through detailed NJ Person Centered Planning Tool and NJ Individual Service Plans, designed uniquely to each person, ALWAYS putting your wants and needs first!
When you join our family, you will become the newest member of a tightly knit group. Our employees will work endlessly to ensure that not only you, but your families as well, identify and help you gain access to the supports you need. There is no such thing as an unobtainable feat or an unreachable dream. When there is a will, there is a way - our job is to find that way.

Assist individuals and their families in the person centered planning process within 10 days of assignment
Work with individuals, their families, and mentors to identify outcomes and utilize their budgets to achieve those outcomes
Assist in identifying services and supports that will achieve the individual's stated outcomes and can be accessed from a provider who is appropriately qualified by the Division of Developmental Disabilities (DDD) to provide those services
Provide a complete and approved ISP within 30 days of assignment
Distribute approved ISP'S to all service providers and team members within 3 days of approval
Monitor service provision every 30 days at a minimum
Write and or review annual ISP for approval
A resident of the state of New Jersey
21 years of age or older
Living in your own home or with family
DDD and Medicaid eligible
Able to meet service specific criteria
